Heparin

I am sure we all have seen the recent media stories about the misuse of Heparin in the pediatric population. Breaking news today revealed the death of two infants in Texas where Heparin is alledgedly  the culprit.

If this turns out to be true, it is a tragedy because it is preventable. Do we need to dramatically change labeling of the different doses of Heparin or take pediatric Heparin out ot the central pharmacies and keep it in NICU and pediatric wards only or do we need to computerize to help prevent mistakes? Or do we need to do all of the above?

I am sure there are many good ideas on how to prevent further accidents with the wrong dose of Heparin. Let us hear from you.
